Home Care Services
Medicaid can help with the cost of personal care and home health care services for financially and medically eligible people.
Approval requires a medical needs assessment, a financial eligibility determination and an authorization from the Department of Social Services.

Already Enrolled in Medicaid:
If you are already enrolled in Medicaid and are in need of home care and Managed Long Term Care services, a determination will have to be made about your level of coverage and health needs.
If you are on a Medicaid Managed Care Plan:
- Contact your plan provider to see if you qualify. More information is also available online from New York Medicaid Choice.
If you are enrolled in Medicaid but not a Medicaid Managed Care Plan:

Home Care Programs
Below are some of the home care services available to Medicaid enrollees:
- Assists with the activities of daily living:
- personal hygiene,
- dressing and feeding,
- household tasks and nutrition.
- May be combined with other services such as visiting nurse or hospice.

Allows eligible Medicaid members to choose and hire their own personal caregiver. This can include a friend or family member, as long as they are NOT the Medicaid member's spouse, their designated representative or the parent of a CDPAP consumer under the age of 21.

Helps people who are chronically ill or have disabilities and who need health and long-term care services, such as home care or adult day care, stay in their homes and communities as long as possible.

The MLTC plan arranges and pays for a large selection of health and social services, and provides choice and flexibility in obtaining needed services from one place.

- Serves people who are medically eligible for nursing home placement but in a less medically intensive, lower cost setting
- Care needs are met by combining the services of an adult home or enriched housing program with supportive home care services.

- Provide services such as nursing services, physical therapy or home health aide services provided at home rather than in a hospital or nursing home.

- A telephone electronic monitoring device that summons emergency assistance to homebound individuals.
- This device is used in conjunction with Personal Care or Long Term Home Health Care Services.
- Designed to help parents care for their severely physically disabled child at home by providing medical assistance, case management, and respite care services to children aged 0-18 who would be Medicaid-eligible if institutionalized.
- May be provided, as medically needed, to a medical assistance patient, in the patient’s home, when the patient is in need of continuous nursing care beyond that available from a home health agency.
